Care Compass is Aeroflow Health's comprehensive Health-Related Social Needs (HRSN) program, designed to identify, enroll, and connect patients to a wide range of benefits based on their unique eligibility.
The program aims to support individuals navigating barriers related to housing, food insecurity, transportation, financial instability, and more—ultimately improving whole-person health.
Our Care Team

Hillary Gale
Hillary Gale serves as the Community Health Program Manager at Aeroflow Health, where she supports initiatives that address Health-Related Social Needs through patient-centered, community-informed strategies. With a strong background in community health and a Master of Public Health from the University of South Florida, Hillary's work focuses on reducing barriers to care and improving health outcomes through direct patient outreach, resource navigation, and strategic program development. Hillary is passionate about advancing equity and designing systems that put people first.

Kecia Phipps
Kecia Phipps is a healthcare professional dedicated to improving health outcomes by addressing social determinants of health. She currently serves as a Community Health Worker at Aeroflow Health, where she works with patients to identify barriers to care and connects them to essential resources such as housing support, transportation, food assistance, and healthcare services.
With a background in community health and a Master's in Healthcare Administration from Florida Atlantic University, Kecia is passionate about advancing health equity through community-based solutions. Her work focuses on bridging gaps between healthcare systems and community resources to ensure individuals receive coordinated, patient-centered support.
